We’re looking for an experienced Primary Care Nurse or Nurse Practitioner to join us as a Care Navigator — a pivotal role at the heart of our service. The Role This is not a traditional nursing role. As a Care Navigator, you’ll be the first point of contact for patients, combining clinical expertise with triage, coordination, and relationship-building. You’ll guide patients through their healthcare journey, ensuring they feel supported and understood at every step. You’ll work closely with GPs and senior clinicians, while helping shape how a modern, digitally enabled practice operates. Key Responsibilities Patient Care & Navigation

Act as primary contact, delivering a personalised patient experienceTriage queries, assess risk, and escalate appropriatelyCoordinate appointments, referrals, tests, and follow-upsSupport patients navigating NHS and private pathwaysProvide continuity, reassurance, and advocacy Clinical Coordination

Work alongside clinicians to deliver safe, joined-up careEscalate clinical concerns appropriatelyMaintain accurate records in line with CQC and GDPR Operational Development

Registered Nurse or Nurse Practitioner (or equivalent AHP)5–10 years’ experience, ideally in primary careStrong triage and clinical assessment skillsPatient-centred, empathetic approachExcellent communication and relationship-building abilityExperience in NHS primary care and/or private settingsUnderstanding of CQC, confidentiality, and GDPRComfortable using digital toolsAble to balance clinical and administrative work Personal Attributes
