The Non-Weight Bearing Pathway is a countywide service supporting patients following a fracture from hospital discharge through to clinical optimisati...

  • Coordinate and support patient flow through the Non-Weight Bearing Pathway, ensuring timely progression through rehabilitation and discharge planning.
  • Arrange and coordinate initial patient assessments and follow-up interventions.
  • Provide rehabilitation interventions within agreed competencies, including progressing exercise programmes and supporting patients to achieve their rehabilitation goals.
  • Assess, order and issue low-level equipment and mobility aids, ensuring safe and appropriate use.
  • Maintain oversight of patient caseloads and waiting lists, ensuring records are accurate and up to date.
  • Liaise regularly with fracture clinics, major trauma centres and other healthcare professionals to obtain clinical updates and support patient care.
  • Communicate effectively with patients, relatives, carers and partner agencies through telephone, virtual platforms, written correspondence and face-to-face contact.

Car ownership and the ability to drive is a requirement of this role as this is a community post.

Knowledge and Experience

Essential

  • Functional skills in literacy and numeracy to Level 2 or equivalent.
  • Completion of an NVQ Level 4, Foundation Degree, or equivalent experience in a health-related subject
  • Evidence of continuing professional development and a commitment to maintaining a CPD portfolio

Desirable

  • Experience of working within a therapeutic physical health setting, preferably within the NHS
  • Experience of applying rehabilitation knowledge and skills in practice

Skills and Abilities

Essential

  • Ability to communicate effectively with a wide range of patients, carers, colleagues and partner organisations
  • Ability to maintain accurate and timely clinical records
  • Ability to work independently within agreed competencies and with minimal supervision
  • Ability to prioritise workload and manage competing demands effectively.

Desirable

  • Experience of using SystmOne and other NHS electronic systems to support patient care and service delivery.
