As a key member of the Pharmacy Homecare Team, you will support the delivery and ongoing development of the Trust's Homecare Medicines and OPAT services.

This includes coordinating homecare prescriptions, patient registrations, renewals, orders, and records, while ensuring compliance with relevant legislation, policies, and professional standards.

You will act as a point of contact for patients, homecare providers, clinical teams and other stakeholders, responding to enquiries and helping to resolve service, delivery, and medication related issues. You will contribute to the monitoring of homecare provider performance, investigate incidents and complaints, and support service improvement initiatives.

The role also includes ordering, dispensing and accuracy checking medicines, supporting medicines reconciliation activities, and counselling patients. You will maintain accurate records, contribute to audit and governance activities, and support service improvement initiatives.

Working closely with pharmacists and multidisciplinary teams, you will help deliver high quality, patient centred pharmacy services while supporting the training and development of colleagues and maintaining high professional standards.

Qualifications

Essential: - Registered Pharmacy Technician with the General Pharmaceutical Council; - Accuracy Checking qualification Level 3

Desirable: - Level 4 qualification in medicines management or equivalent, or working towards

Experience

Essential

  • Experience of medicines management/optimisation
  • Minimum 1 years experience as a qualified pharmacy technician
  • Ability to cope with large unpredictable workloads in a timely, efficient and reliable manner

Desirable

  • Experience of hospital pharmacy, homecare and OPAT
  • Experience of financial processes
  • Clinical ward based pharmacy experience
  • Experience of complaints and incident processes

Skills

Essential

  • Good communication skills
  • Attention to detail
  • Good level of experience of IT software, in particular good Excel spreadsheet skills and Pharmacy IT system
