  • Deal with all aspects of referrals, working with the clinicians and Administration Hub Lead
  • Ensure all referrals are sent within the timeline the referral is requested.
  • To complete all Advice and Guidance for clinicians
  • Maintain a referral data base and support audits on referrals.
  • Read code relevant information in relation to the patients care from the patients clinical letters, into the clinical computer system.
  • Summarise clinical notes
  • Deal with day-to-day post including electronic post and discharges; scan the post directly onto the computer system.
  • Answer the telephone at busy periods when requested to support the reception team to provide safe patient access to the service.
  • Ensure total familiarity with all appointment systems including regular and incidental variations
  • To provide administrative support and services to all members of the practice team including reception.
  • Follows the company standards for ethical business conduct

To work closely with the clinical team and Administration Hub Lead, providing secretarial and administrative support to ensure the smooth running of the practice and the delivery of high-quality patient care.

Main Duties and Responsibilities- Manage all aspects of patient referrals, ensuring they are processed within the required timescales.

  • Complete Advice and Guidance requests on behalf of clinicians.
  • Maintain referral databases and support referral audits.
  • Read-code relevant information from clinical correspondence and accurately update patient records.
  • Summarise patients' clinical records.
  • Manage incoming and outgoing correspondence, including electronic post, hospital discharges and scanning documents onto the clinical system.
  • Support the reception team by answering telephone calls during busy periods when required.
  • Maintain a good understanding of the practice appointment systems and any changes.
  • Provide administrative and secretarial support to all members of the practice team.

Confidentiality and Information Governance- Maintain strict patient confidentiality and comply with data protection requirements.

  • Follow all relevant practice policies and procedures.
  • Ensure patient information is only shared with authorised individuals.

Health and Safety- Comply with the practice's health and safety and infection control policies.

  • Identify and report potential risks and hazards.
  • Maintain a safe and tidy working environment.

Personal and Professional Development- Complete all mandatory training as required.

  • Participate in training, meetings and annual performance reviews.
  • Take responsibility for ongoing learning and professional development.
  • Contribute to improving practice processes and the quality of services.

Communication and Teamwork- Communicate effectively with patients, carers, clinicians and colleagues.

  • Work collaboratively with the clinical and administrative teams.
  • Respect equality, diversity, dignity and the individual needs of patients and colleagues.
  • Contribute positively to the wider practice team.

General Responsibilities- Manage workload effectively and meet deadlines.

  • Maintain high standards of accuracy and quality in all administrative duties.
  • Participate in audits and support service improvements.
  • Follow practice policies, procedures and professional standards.
  • Undertake other reasonable duties appropriate to the role.
