- In collaboration with the Lead for the safeguarding team /Named Nurse for Adult Safeguarding and the multi-disciplinary team you will be responsible for the delivery of safe, effective care leading to an excellent experience for patients and carers by
- Being responsible for ensuring high quality, evidence-based patient centred care is delivered and providing assurance of this whilst delivering the Trusts core objectives and strategies.
- Developing and delivering continuous improvement initiatives to ensure that the service meets all quality indicator targets, key performance indicators and quality and safety standards and initiatives relevant to the service.
- Deliver the learning requirements to staff in the trust through planned education activities in line with the learning disability and autism code of practiceo Reviewing and implementing the management of patient pathways to improve the quality of care and efficiency in the wards/departments, ensuring the patients experience is positive while under our care.
- Leading on the trust's clinical practice standards, clinical improvements and/or strategies for the delivery of excellent care for patients with learning disability, autism or dementia and subsequent training and education requirements
Work in partnership with individuals, families and carers, using a range of assessment methods as appropriate (e.g. history-taking, holistic assessment, identifying risk factors, mental health assessments, requesting, undertaking and/or interpreting diagnostic tests and conducting health needs assessments).
Challenge practice so that the best and most appropriate treatment is given to the patient, maintaining and monitoring high standards of care, recognising and responding proactively to patients changing clinical needs
Working collaboratively, managing a group of patients to deliver high quality skilled, evidence based and patient centred
Be fully conversant and wholly accountable for your practice and maintaining your professional registration in line with the Nursing and Midwifery Councils (NMC) Code
Recognise the limits of individual competence and knowledge and undertake and complete further training and academic qualifications as relevant to the role and service requirements.
Develop a culture of providing and accepting constructive feedback and use this to reflect on your own practice and that of others.
Communicate clearly and appropriately with patients, carers/ relatives and all other members of the multidisciplinary team
Continuously assess and evaluate the communication, educational and information needs for patients and their families and devise plans to ensure needs are met; ensure instructions/information is understood.
Participate in the development and implementation of a robust clinical audit programme as well as participation in the clinical governance agenda in line with the risk management strategy and policy
Contribute and participate towards patient engagement and involvement initiatives e.g. patient focus groups and patient information sessions.
Contribute to the training needs and gap analysis of the service and provide training and education to meet the service requirements.
Act as a clinical role model and demonstrate specialist knowledge and high standards of clinical practice.
