- Provide person centred, outcome focussed, support plans and risk assessments for the people we support; which are accessible and understood by all staff
- Encourage independence and self-management in a safe, secure home environment KM's and strive to restore self confidence in the individual being supported
- Ensure that therapeutic programmes of care as defined by clinical colleagues are understood and implemented as planned
- Organise and maintain satisfactory levels of support by deploying staff as necessary to meet the service needs over a 24-hour period, 7 days per week and 365 days per year
- Liaise with families, care managers, nursing, allied health professionals, the Inreach team and other providers to ensure the highest quality of support is available for each person we support.
- Maintained responsibility for the identification of continuing educational needs and development. Actively participate in the annual appraisal process and formulate a personal development plan
- Line manage the Care Co-ordinators, undertake annual appraisals, supervisions, manage absence and oversee their development
- Act as an assessor for staff undertaking NVQ levels 1 to 3
- To identify training needs of staff and empower staff to access training opportunities, whilst ensuring compliance with statutory and mandatory training
- To organise and lead, where required:
- Team meetings
- Audits
- Quality Reviews
- Referral / Assessments
- Continuing Health Care Assessments
- Customer Care Surverys

Qualifications
Essential
- *QCF Level 4 Level 4 Diploma in Health & Social Care or willing to work towards it
- *Good literacy skills
- *Good numeracy skills
Desirable
- *Basis management qualification
- *Previous management experience
- *ECDL or equivalent
