The role offers a unique combination of street-based outreach, clinical assessment, treatment, health promotion, care coordination and system navigation. The postholder will support individuals with complex physical health needs, mental health challenges, neurodiversity, substance misuse issues and safeguarding concerns, delivering care in a way that is accessible, responsive and tailored to individual circumstances.

A key part of the role will be building trusted and meaningful relationships with people who may have experienced repeated trauma, stigma and exclusion from services. As a valued member of a growing inclusion health network, the postholder will contribute to the continued development of an integrated inclusion health model across rough sleeping, homelessness, health and social care services. This is an opportunity to help shape a pioneering service in Hounslow, influencing how partners work together to deliver more coordinated, equitable and effective support for residents with the greatest health needs.
