The role will involve supporting both the Discharge to Assess model to enable people to leave hospital as well as supporting people with complex needs through the completion of Care Act Assessments with an understanding of all relevant legislation and Continuing Healthcare. We promote personalisation, focusing on the wellbeing of the individual. Through care planning we strive for increased independence, choice and control by enabling people to direct their own support. You will gain extensive experience in the field of adult social care, safeguarding practice and multi-disciplinary working. You will have the ability to undertake innovative assessments and care planning with individuals, carers and their personal support networks.We promote personalisation focusing on the wellbeing of the individual.
