You will work as part of the Tier 3 Specialist Multi-Disciplinary Team, providing expert clinical input and leadership for diabetes medicines optimisation across neighbourhood and community services. The role will involve supporting patients with complex diabetes and multiple long-term conditions, developing and implementing medicines pathways, and promoting safe, evidence-based prescribing practice.
As the specialist pharmacy lead within the programme, you will provide expert advice and education to healthcare professionals across primary care, including community pharmacists, practice pharmacists and PCN teams. You will act as a key link between clinical practice and service development, helping to design and evaluate innovative approaches to diabetes care.
The successful candidate will have substantial experience in diabetes management, advanced clinical pharmacy practice, and leading service improvement. You will be confident working across organisational boundaries, influencing practice, and supporting multidisciplinary teams to deliver excellent patient-centred care.
The post offers a flexible hybrid working approach, with time spent in neighbourhood clinical settings, GP practices, community venues and home working, depending on service need.
