Healthcare Support Worker - Spinnaker Ward

• To actively assist in the delivery of individual patient rehabilitation programmes (for example, activities with service users) under the direction of registered nursing staff and/ or the therapy team, planning, organising and delivering clinical care in line with patient and service need.
• Work as an effective member of the multidisciplinary team.
• To follow instructions and report any changes in patients' progress or medical condition.
• The post holder will be required to undertake a range of clinical and clerical activities as delegated by the nursing and or therapy team.
• Staff will be required to provide cover on sister wards as per the needs of the individual service.
• To contribute to the provision and monitoring of high-quality individualised nursing care and rehabilitation, in accordance with agreed policies and procedures
• To utilise an appropriate level of knowledge, understanding, skills, attitudes and behaviours and work within their scope of practiceThe job will includes working internal rotation over the 24 hour care period, shift times:

• Will communicate on a regular basis with the patients, their relatives and colleagues in a caring, compassionate and professional manner using a trauma informed approach.
• Provide support to the nurse in charge and/or the person responsible for the Clinical care of the patient
• Exchange factual information with patients using tact, reassurance, empathy, compassion and acknowledging barriers to understanding.
• Use a range of skills to address barriers to understanding eg. a patient has a physical impairment, mental health condition or learning disabilities
• Promote and develop good relationships with patients, staff and visitors
• Promote good relations between the departments and services
• Respect confidential information obtained in the course of duty and act in a manner to respect the customs, individuality, values, sexuality and spiritual beliefs, activity, and supporting the individual to fulfil these.
• Demonstrates an ability to input into IT systems as required.
• Identifies and minimises interpersonal conflict.
• Recognise potential deterioration and appropriately escalate, working to maintain safety until assistance arrives.

• To be aware of the potential for vulnerability and abuse amongst the patient and family/carer and using the local policy to inform appropriate personnel in line with Protection of Vulnerable Adults (POVA), Child Protection/Safeguarding and HIOW Policies.
• Maintain personal and professional boundaries at all times.

• Work under the direction and supervision of the Registered / Healthcare Professional.
• Able to complete a range of routine work procedures and practices that require a base level of theoretical knowledge
• Knowledge of care and related procedures, clinical observations, relevant legislation, including Mental Capacity Act
• Attend short courses to undertake patient and clinical care duties
• Completion of competencies with equivalent relevant experience to vocational qualification at level 3

• Able to implement clinical care plans as directed by the Registered Nurse/ Registered Healthcare Professional
• Undertakes a limited range of delegated clinical care duties in accordance with the specified plan of care and provide feedback to the Nurse in charge/ Registered Healthcare Professional
• Establish therapeutic relationships with individual patients, their relatives and supporters as appropriate.
• To provide the necessary physical care to patients in in-patient settings to help them complete daily living activities, i.e., personal hygiene, diet and fluid intake, clothing and laundry, personal environment, elimination, maintaining rest and sleep and activity, physical observations
• Able to record patient observations relevant to clinical setting, for example, but not limited to:

• To be involved in judgements involving facts or situations that require some analysis
• Assess a patient's condition through observation/test results
• Assess comfort of patients
• Initiate emergency procedures if situation arises
• Able to escalate concerns to a registered nurse/practitioner appropriately
• Help to maintain a safe, therapeutic environment for patients, staff and visitors to the ward to reduce the likelihood of accidents, untoward incidents or clinical incidents.
• Engage with patients in meaningful activity, involving them in a wide range of therapeutic interventions. These may be therapies suggested by registered staff as well as involving patients in everyday living activities.

• Participate in audits, surveys, research and development activities as required
• Carrying out duties delegated by the registered staff

• Contributes to updating patients' records, for example, recording care provided, observation records, position charts, food and fluid charts
• Use of information technology to benefit personal development and patient care
• Maintain IT and manual records of patient contacts as per Trust policy.
• Input and update various patient records from other agencies/departments

Essential

• Level 2 qualification or equivalent in a relevant area gained through experience, on the job training, short courses and completion of work-based competencies
• Completed care certificate, in the process or willingness to work towards the care certificate.
