- Supporting the delivery of continence assessments and personalised care plans
- Carrying out basic clinical tasks in line with competency frameworks
- Providing education, advice, and reassurance to patients and carers
- Assisting with continence product provision and reviews
- Maintaining accurate electronic patient records
- Working collaboratively within a supportive multidisciplinary team
- Previous experience as a Health Care Assistant (Band 2 or 3)
- A caring, patient-focused approach and excellent communication skills
- Confidence working independently in community settings after training
- Willingness to develop enhanced skills in continence care
- Ability to travel throughout Northamptonshire
- Full training and ongoing professional development
- Supportive team environment with specialist nurse mentorship

Experience
Essential: - Previous autonomous experience in a home visiting caring environment/role
Desirable
- Ability to deal with non-routine and unpredictable nature of workload and individual patient/service user contact.
- Understanding of practices that promote self-care management, behavior change and independent living.
Skills
Essential: - Effective written and verbal communication skills; - Clearly demonstratable IT skills
Desirable: - Ability to carry out assigned tasks effectively in a busy environment; - Experience of documenting on SystmOne
