- Your main duties will include providing a number of clinical activities and holistic care under the direct delegation of a registered nurse. And carers using a wide range of clinical skills and interventions within a home environment.
- You will need to possess excellent Communication skills and a team player and be able to adapt to the changing work patterns of the intergrated community nursing team by remaining calm under pressure. You will also need to have good IT skills to support the systems in place.
- The Integrated Community Nursing Services has a key focus on facilitating discharges, case management and reducing admission to hospital. The service works closely with Acute Hospitals, Hospice, including GPs. The service provides care to patients in the community setting.
- You will be well supported by our current team and provision is also made for your continued professional development.

Qualifications
Essential
- Higher Professional Diploma in Health and Wellbeing (NVQ/QCF - Level 3) or a recognised equivalent course
- Willingness to undertake further training as appropriate
Desirable
- GCSE grade 9 - 4 or equivalent in English and Maths
- Registration with the national regulatory body (when it becomes available).
Experience
Essential
- Experience of working in a health or social care setting
- Experience of working as part of a team
- Experience with the use of Microsoft Office and a variety of databases.
Desirable
- Experience of working as part of a team
- Experience with the NHS patient information database (Rio, CIS)
Skills & Knowledge
Essential
- Knowledge of NHS Demonstrate an acceptable level of numeracy and literacy. An awareness of risk management.
- Knowledge of multi-agency services available and processes for referral
- Knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being
Desirable
- Knowledge of local health needs strategies.
