The post holder will have professional responsibility for a defined population/caseload and lead the delivery & organisation of the public health program. Delivering the nursing component of the Healthy Wales Child Programme to a 0-5 age population within a defined area.
Based within the community setting, the practitioner is responsible for the assessment, planning, implementation and evaluation of care offered to families and external organisations, with particular attention to safeguarding children.
They will be responsible for coordinating and delegating work within a skill mix team.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

- SCPHN Health Visitor Post graduate diploma or Certificate or working towards it.

- In-depth understanding of Public health agenda & priorities

- Healthy Child Wales Programme(2015)
