Health Coach

• Use the principles of Making Every Contact Count (MECC) to provide brief health and wellbeing interventions, to help people make informed choices and take positive steps to improve their health.
• Work with individuals to identify health and wellbeing needs and develop personal health plans and realistic action plans, reviewing progress and adapting goals to help people make and sustain positive behaviour changes.
• Plan

• Carry out basic health checks and observations, such as height, weight and blood pressure.
• Document health plans and maintain accurate patient records.
• Identify individuals who may benefit from additional support and signpost or refer them to appropriate health, wellbeing and community services.
• Build positive, supportive and non-judgemental relationships with individuals, carers and families, adapting communication to meet different needs and circumstances.
• Work collaboratively with colleagues, health professionals and external organisations to provide coordinated support and improve outcomes for individuals.
• Develop and maintain links with local community, voluntary and health organisations, including identifying new referral pathways and strengthening existing pathways into the Health Trainer Service.
• Collect data to contribute to the evaluation and improvement of service performance.
• Recognise and appropriately respond to risks or concerns relating to patient care, escalating issues to senior colleagues where required.
• Plan and manage your own workload effectively, ensuring agreed activities, appointments and targets are delivered within required timescales.
• Attend and contribute to relevant meetings, training, community activities and health promotion events.
• You must be able to travel across the city to meet service users and deliver sessions in a range of community settings.
• You must becompetent in phlebotomy, or be willing to develop this skill and achieve competency within three months of appointment.

Essential

• Excellent verbal and written communication skills, with the ability to communicate confidently and sensitively with individuals, groups, colleagues and partner organisations.
• Ability to work independently and use initiative, with strong organisational and time management skills to prioritise workload, manage competing demands and organise appointments, activities and events.
• Confident in using IT and different software including Microsoft Office, Outlook Calendars. Patient management systems such as SystmOne would be beneficial.
• Knowledge and understanding of local health, public, voluntary and community services, particularly those in Sheffield, and how people can access appropriate support.

Essential

• Experience of maintaining accurate and confidential records, with an understanding of and ability to work within data protection and confidentiality requirements.

Desirable

• Experience of working with individuals and groups from a wide range of backgrounds and with different health and wellbeing needs.
• Experience of using motivational and behaviour change approaches to support people to make positive and sustainable lifestyle changes.

Essential

• Competency is Phlebotomy or willing to complete training within 3 months.

Desirable

• Level 3 Health Trainer certificate
