- To provide basic nursing, therapy and social care to people and carers, carrying out delegated clinical, therapeutic and personal care tasks
- To promote the physical, mental and emotional health and wellbeing to individuals and client groups.
- To be an effective member of the multi-disciplinary team.
- Communicate effectively with patients and carers, recognising their needs for alternative methods of communication.
- To carry out care/other duties under direct supervision of a registered professional.
- To support the band 6 nurse with tasks in patients homes.
Have an understanding of support services available in the community
- Maintain excellent written patient records, and demonstrate the principles of patient confidentiality in accordance with Caldicott guidelines.
- Recognise and respond to challenging behaviour.
To maintain close liaison with colleagues and to communicate appropriately with other departments/services as required. Ensuring effective feedback regarding patients conditions to appropriate health care professionals
If successful this vacancy will require an enhanced DBS check at a cost of £54.90. This cost will be deducted from your first Trust salary
In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16

Knowledge and qualifications
Essential
- Mathematics GSCE or equivalent experience
- English GSCE or equivalent experience
- Literate Numerate
