The successful candidate is expected to work and document to a high standard and work in a way that reflects the DCHS values.

To maintain standards, ensuring quality of patient care through the development of a range of knowledge and competencies to support clinicians.

To assist all clinicians in delivering agreed programmes of treatment and care where appropriate and work as an effective member of the wider multi-disciplinary team.

To carry out individual care and reablement without direct supervision in the community.

To provide support to patients to optimise their health and independence.

Be willing to support Perth House in Chaddesden when and if staffing cover requires.

The Purpose and Activities of DCHS?

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

Education, Qualifications and Training

Essential

  • GCSE Maths & English (C/4 or above) or equivalent
  • Educated to NVQ 3 level (or equivalent) in a relevant subject or equivalent previous proven experience
  • IT skills

Experience and Knowledge Required

Essential

  • Experience providing personal care

Desirable

  • Previous experience in a health & social care setting
  • Understanding of the health and social care system

Skills and Attributes

Essential

  • Excellent communication and interpersonal skills
  • Ability to work in a Multidisciplinary team
  • Willingness to travel within a locality
  • Flexibility, adaptability and Resilience

  • The ability to Recognise and escalate deteriorations in physical and mental health
