HCC Surveillance Pathway Navigator

To engage with patients undergoing HCC Surveillance due to their Liver Disease. Support scheduling of clinic and imaging appointments to ensure surveillance schedule is maintained. To coordinate the necessary appointments, or investigations to expedite patients with abnormal results (e.g. suspected HCC) and ensure the correct onward pathway is followed in a timely manner. Make telephone calls to patients to remind them of upcoming scans / laboratory tests; identify and assist in resolving any barriers to engagement; coordinate text reminders for surveillance appointments for patients. Support patients with additional needs to attend appointments - for example, meet at front door / attend appointment with patients.

Coordinate and provide administrative support to the clinical team to ensure all relevant paperwork and clinical information is available for consultations. Maintain and populate the patient tracking list ("Database") for the surveillance clinics and scans, adding newly referred patients. Document and monitor aspects of care coordination and service delivery for audit and reporting to the Hepatology team and the Humber and North Yorkshire Cancer Alliance. Respond in a timely manner to enquiries from the patient's GP regarding liver disease stage, care and treatment on the cancer surveillance pathway.

Main duties and responsibilities of post holder

Patient Care / Clinical Services

Responsibility for managing own time and ensuring tasks are completed in the required timeframe. Work autonomously in ensuring patients are tracked and progressed along their pathway accordingly.

Document and monitor aspects of care coordination and service delivery for audit and reporting to the Hepatology team and the Humber and North Yorkshire Cancer Alliance.

Support scheduling of clinic and imaging appointments to ensure surveillance schedule is maintained.

Coordinate and provide administrative support to the clinical team to ensure all relevant paperwork and clinical information is available for consultations.

Maintain and populate the patient tracking list (Database) for the surveillance clinics and scans, adding newly referred patients.

Identify from the tracking list the next steps in the patients surveillance pathway in line with target surveillance dates and or clinical need.

Develop an understanding of the milestones within the Liver Cancer Surveillance pathway and relevant national guidance (e.g. NICE / BSG cirrhosis guidelines / Minimum standards for HCC surveillance).

Respond in a timely manner to enquiries from the patients GP regarding liver disease stage, care and treatment on the cancer surveillance pathway.

Encourage and support active and healthy lifestyle choices.

Monitor patients on surveillance pathways and proactively find resolutions to improve adherence to target surveillance dates, e.g. tracking attendance and DNAs, escalation of overdue patients to the Lead Nurse / clinical team. Work with lay peer support workers to maintain engagement and reengage with patients with surveillance following a DNA. Proactively finding resolutions to barriers to care.
