Clinical Practice & Extended Role Delivery

  • Deliver specialist clinical services within the GPwERs defined extended role (e.g. Dermatology, MSK, Neurology, Headache, Minor Surgery, Epilepsy, etc.).
  • Manage acute, chronic, and complex conditions, including those requiring enhanced expertise.
  • Undertake specialist clinics, procedures, or assessments relevant to the extended role.
  • Ensure appropriate investigations, referrals, and follow-up, including triaging referrals within the specialist area.
  • Prescribe in accordance with local and national guidelines, ensuring safe medicines optimisation.
  • Maintain accurate, timely, and complete patient records.

Performance, Quality & Governance

  • Work within Care Quality Commission (CQC), General Medical Council (GMC), and NHS England frameworks.
  • Participate in clinical audits, research, and quality improvement initiatives, particularly within the extended role area.
  • Contribute to development of clinical pathways and protocols relevant to the specialty.
  • Participate in significant event reviews and complaints handling.
  • Maintain compliance with information governance, GDPR, and confidentiality standards.

Communication & Multidisciplinary Working

  • Undertake patient consultations via face-to-face and telephone as appropriate.
  • Provide specialist advice and guidance to colleagues within the practice, PCN, or wider system.
  • Work collaboratively with multidisciplinary teams including nurses, allied health professionals, pharmacists, and secondary care specialists.
  • Participate in MDT /CPD meetings, case discussions, and clinical reviews, particularly within the specialty area.
  • Promote health education, prevention, and self-management strategies.

Leadership, Education & Service Development

  • Contribute to the development and improvement of specialist services within the practice, PCN, or wider system.
  • Provide teaching, mentorship, or upskilling to colleagues in the specialist area.
  • Contribute to innovation and service redesign to improve patient outcomes and access.

Professional Development

  • Maintain competence in both core general practice and the extended role specialty.
  • Engage in Continuous Professional Development (CPD), appraisal, and revalidation.
  • Maintain evidence of training, accreditation, or experience relevant to the GPwER role.

  • Promote a holistic approach to patient care, addressing physical, psychological, and social needs.
  • Uphold safeguarding responsibilities in line with national and local policies.
