• Deliver high-quality, patient-centred nursing care in a primary care setting.
• Undertake routine nursing procedures including immunisations, cervical screening, wound care, venepuncture, ECGs, and health promotion.
• Support the management and monitoring of patients with long-term conditions.
• Provide health education, preventative care advice, and support patients to improve their health and wellbeing.
• Maintain accurate patient records and ensure care is delivered in line with NMC standards, practice policies, and clinical guidelines.
• Work collaboratively with GPs and the wider multidisciplinary team to deliver safe and effective patient care.
• Participate in quality improvement activities, clinical audits, and QOF-related work.
• Maintain infection prevention and control standards and ensure the safe management of clinical equipment, vaccines, and supplies.
• Support safeguarding responsibilities and recognise concerns relating to vulnerable adults and children.
• Engage in ongoing professional development, training, clinical supervision, and appraisal.

• Provide safe, effective, and evidence-based nursing care to patients within the practice and, where required, in patients' homes.
• Deliver a range of clinical services including immunisations, cervical screening, wound care, venepuncture, ECGs, specimen collection, travel health, contraception services, and health promotion.
• Support the management and monitoring of patients with long-term conditions and contribute to disease-specific clinics and recalls.
• Assess patient needs, provide appropriate advice and education, and refer to other healthcare professionals when required.
• Maintain accurate clinical records and ensure confidentiality at all times.
• Participate in Quality and Outcomes Framework (QOF) activities, clinical audits, quality improvement initiatives, and service development.
• Work collaboratively with GPs, nurses, healthcare assistants, and the wider healthcare team to deliver high-quality patient care.
• Maintain infection prevention and control standards and ensure the safe management of vaccines, medications, and clinical equipment.
• Support safeguarding responsibilities and recognise concerns relating to vulnerable adults and children.
• Participate in practice meetings, mandatory training, clinical supervision, appraisal, and ongoing professional development.

Essential

• Fully trained and experienced General Practice Nurse.
• Minimum of 2 years' recent experience working as a Practice Nurse within a GP surgery/Primary Care setting.
• Competent and confident in cervical cytology (smear tests).
• Competent and confident in childhood and adult immunisations, including vaccine administration and cold chain awareness.
• Experienced in chronic disease management, including long-term condition reviews (e.g. diabetes, asthma, COPD, hypertension).
• Experienced in complex wound care, including assessment, management and dressing of complex wounds.
• Able to work independently and manage a clinical workload safely and effectively.
• Excellent communication, clinical documentation and patient-care skills.
