To deliver specialist frailty care and interventions, including assessment, treatment, and the management of a defined caseload as part of the multidisciplinary team. Knowledge skills and training
Essential: - Registered Practitioner; - Evidence of recent professional development; - Experience of working in community
Desirable
- Physical examination qualification
- Experience at band 6 level, or above
- Able to use own initiative and appropriate risk taking
- Experience and knowledge of working with frail patients
Job specific experience
Essential: - Experience of managing change; - Demonstrable knowledge of assessment and therapeutic interventions in area of specialism
Desirable: - Experience at band 6 or above; - Recent previous experience within a comparable community role
Managerial/ Supervisory experience
Essential: - Experience of providing clinical supervision and mentoring to junior staff; - Experience of supporting and supervising learners
Desirable: - Experience of devising and delivering training
Information Technology / Resources
Essential: - Experience of using electronic patient / service user record systems
Business Travel
Essential: - Subject to the provisions of the Equality Act, able to travel using own vehicle on Trust business.

Essential
- Demonstrable skills in written and spoken English to a standard which enables the post holder to carry out the full range of duties and responsibilities of the role effectively.
