Floating Support Worker

Build effective relationships with referral agencies to support new referrals into the serviceDevelop and deliver tailored support and safety plans for service usersMaintain regular contact with service users in line with contractual requirementsSupport individuals to identify and achieve their personal goalsDevelop independent living skills, including support with domestic tasks and daily livingEnsure properties are well maintained and comply with relevant support agreementsSupport effective rent collection and minimise void periodsMaintain accurate records and ensure support is delivered in line with service requirementsWork collaboratively with partner agencies and other professionals to achieve positive outcomes What We're Looking For

Previous experience working as a Support Worker or in a similar roleExperience within housing support, social care or supported accommodationKnowledge of support planning, safeguarding and independent livingStrong communication and relationship-building skillsA proactive and person-centred approach to supporting vulnerable individualsAbility to work independently across dispersed accommodationRelevant qualifications in social care or a related field are desirableFull UK driving licence is essentialEnhanced Child and Adult DBS is required
