The post holder will provide high quality, patient centred care for frail, housebound and care home patients, undertaking home visits where indicated and supporting patients with frailty, complex needs and multiple long term conditions. Duties include monitoring conditions, care planning, identifying deteriorating patients, coordinating interventions and working with GPs, community services, social care and voluntary sector partners, plus health promotion, screening, safeguarding, advocacy, referral management and meeting quality targets.
The post holder will lead the Neighbourhood Frailty Team, offering supervision and mentorship to staff, undertaking advanced assessments, supporting complex decision making, leading the management of patients requiring community interventions and promoting evidence based practice and quality improvement.
As line manager, the post holder will support staff performance, wellbeing and development through supervision, audits, competency assessment, development planning and attendance management, monitoring standards and acting as key contact for staff.
The role is responsible for coordinating daily workloads, ensuring effective use of resources, monitoring caseloads and escalating capacity or safety concerns. The post holder will contribute to PCN and neighbourhood frailty service development through improvement, clinical governance, innovation and local/national priorities, supporting integrated neighbourhood care and future expansion.

Qualifications
Essential
- Registered General Nurse
- Professional knowledge in primary healthcare acquired through post graduate degree/diploma
- Diplomas in chronic disease management e.g. Diabetes, asthma
- COPD etc
Desirable
- Diabetes PgDip is desired.
- Non-Medical Prescriber Qualification.
- Working towards a level 7 Qualification relevant to the role, e.g. MA Leadership Qualification or other MA level qualification relevant to the role.
Experience
Essential
- Experience of Clinical leadership, line management and supervising
- Registered nurses.
- Experience of providing care to patients within their own home or in a care home setting.
- Significant experience working in a primary care or community nursing setting and has undertaken additional training in the majority of the following areas:
- Long Term Condition Reviews
- Asthma and COPD
- CVD and Heart Failure
- Diabetes
- Urinalysis
- ECGs
- Venepuncture
- Immunisations
Desirable
- Experience of Clinical Leadership of MDTs.
