- 1 WTE Consultant Geriatrician, Senior Health post
They will hold clinical responsibility for half of the patients on this 28 bedded ward. With the multidisciplinary team, the appointee will provide Geriatric Medicine and comprehensive geriatric assessment to older adults with frailty admitted to the ward aiming for rapid, safe discharge home through integrated working with the local community trust.

These postholders will lead reactive frailty care across St George's and Wandsworth & Merton CLCH (Community Trust) by providing clinical leadership and senior decision-making across a joint caseload. They will be supported by an MDT from a range of professional backgrounds including ACPs, GP, nurses, therapists and admin. The role will complement admission avoidance at the front door through close alignment with ED, Senior Health and Acute Medicine departments including Same Day Emergency Care.

Qualifications
Essential
- MRCP or equivalent
- Full registration with the GMC
- Entry on the Specialist Register for Geriatric Medicine, or within 6 months of CCT at time of interview
- Advanced Life Support
Desirable
- MD, PhD or equivalent
Clinical experience
Essential
- 1. Inpatient care of older people living with frailty
- 2. Acute medical take
- 3. Leading a multidisciplinary team
- 4. Experience of integrated care or cross-organisational pathway development for people living with frailty
Desirable
- 5. Experience of working within a community service
Policy and quality
Essential
- Audit and research
- Participation in clinical governance
- Familiarity with service organisation and development
Teaching
Essential
- 1. Understanding of modern medical curricula and of learning assessment tools
- Evidence of maintaining and/or updating teaching skills
Desirable
- 2. Experience in training, teaching and assessing undergraduate medical students
