Main Duties and Responsibilities

  • To support the Discharge Team in facilitating timely discharges for complex patients
  • Provide safe, compassionate and person centred care and support to patients during their transfer from hospital to their community discharge destination
  • Home visits and lone working without direct supervision
  • Transfer patients to the discharge lounge if required prior to supporting on community transport
  • Workload allocated by a registered professional
  • Flexible working pattern to meet the needs of the service
  • Excellent communication skills with patients, relatives and colleagues
  • Have an understanding of services available within the community
  • Maintain excellent written patient records, demonstrating patient confidentiality in accordance with Caldicott guidelines
  • Maintain close liaison with colleagues and to communicate appropriately with other departments / services as required. Ensuring effective feedback regarding patient conditions to appropriate health care professionals
  • Provide support and mobility including personal care, if needed

If successful this vacancy will require a Standard DBS check at a cost of £26.90. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.

Qualifications

Essential

  • NVQ - Level 2 or equivalent experience in caring role
  • GCSE level of education
  • Willing to study for Care Certificate

Desirable

  • NVQ 3 or equivalent

Experience

Essential

  • Experience in patient/customer facing role

Desirable

  • 2 years experience in patient facing role

Skills

Essential

  • Able to communicate in complex situations

Knowledge

Essential

  • Understanding of the NHS

Desirable

  • understanding of NHS discharge process
