Maintains safety, privacy and dignity of all patients in the delivery of patient centredcare, recognising and respecting differences including spiritual and cultural beliefs.Responsible for recognising the signs of patient harm, abuse or neglect, includingpoor clinical practice, identifying issues regarding the safeguarding of children whomay be at risk and reporting all such concerns in accordance with TrustSafeguarding procedures.Undertakes delegated activities within level of own competency, and with dueregard to risk assessment, to assist with the implementation of intervention plans aspart of the multidisciplinary team. Patients have a range of mental healthconditions, may demonstrate behaviours that challenge and may have difficultfamily situations of relevance.Completes patient assessment tools and accurately summarises within patient carerecords.Identifies changes in patients presentation and acts, reports and recordsappropriately.Regularly accompanies and provides support to patients in a variety of communitysettings, as delegated and dependent on current risk assessment. This includessocial inclusion/community integration/transfer to and from appropriate areas e.g.acute hospitals, dentist etc.Accurately records and reports physiological observations (e.g. weight,temperature, blood pressure, pulse), as required.Carries out venepuncture and ECGs as required, if trained.Assists in the provision of health education and advice on health promotion topatients and their carers.Demonstrates safe breakaway techniques in the management of violence andaggression as required.Demonstrates safe moving and handling of patients using equipment as required.Where appropriate and with authorisation, acts as an advocate for patients/carers.

This position has a starting salary of £25,760 which rises to £27,476 per annum after 2 years and a generous NHS pension scheme. Qualifications

Essential

  • Diploma in Health and Social Care level 2 or equivalent
  • Care Certificate
  • Key skills in Literacy and Numeracy level 2
  • Successful completion of Trust's full HCA Framework within 6-month probationary period

Experience

Essential: - Previous experience as a Band 2/3 HCA or equivalent in a care environment.

Desirable: - Previous experience of working with people with mental/social health needs or learning disabilities.

Knowledge and Skills

Essential

  • Person centred care
  • Awareness of legal frameworks including Mental Health legislation (within agreed timescale)
  • Effective verbal and written communication skills
  • Time management and ability to prioritise effectively

Desirable: - Competency in venepuncture; - Competency in ECGs
