An exciting opportunity has arisen to employ a full-time Community Support Worker within the Neighbourhood Mental Health Team. This role will focus on...
Will work as a key member and resource of the MDT core team.
Aspects of the role would also include close working within the
MDT, undertaking and supporting individuals accessing the service in Recovery
focussed activities.
There is scope for professional development as the trust has a
repertoire of training available to those wishing to extend their skill
set. Developing our workforce is

Support registered professionals to deliver evidenced based
interventions, managing a caseload of patients.
Supporting and signposting to improve social isolation and
improved population health.
You will have experience of working with this user group in
community settings.
The ability to work flexibly as the need of the service demands is
Essential.
Attend weekly
MDT meetings, participate in professionals meetings, signpost to adjunct
services depending on patient need.
Support
and guidance to the carers of patients accessing the services.
Act as a role model to colleagues, always seeking to maintain the
highest standards of professionalism.
Use their initiative and take responsibility for themselves and
the quality of their work and the service they provide to patients.
Work across all clinically appropriate patients, 17-years-old and
over, to deliver evidence-based interventions.
Provide support to MDT colleagues in delivering
interventions.
Work as integral team member, bolstering
recovery-based interventions in the community.
Support medicines management/ clinics related to
physical health delivery.
Represent the team on specific projects and
working groups as required.

Qualifications
Essential
- Level 3 qualification (e.g., A-level, NVQ3) in Care or equivalent qualification or experience.
- Level 2 qualification (e.g., GCSE, NVQ2) in Care or equivalent qualification or experience and will be expected to undertake the relevant level 3 qualification on appointment to post.
- Will be expected to meet the Care Certificate standards within 12 weeks of commencing employment with the Trust.
- I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
Knowledge
