Our aim is to provide treatment and support to patients in their own homes. Whilst undertaking a holistic nursing assessment, we aim to encourage patients to remain as independent as possible, in order to prevent hospital admissions.
Due to the nature of this role, lone working and the ability to meet the travel requirements of the post are essential.
Main duties of this role include, but are not limited to;
- Palliative care
- Wound care
- Managing end of life symptoms and providing support for relatives/carers- Managing long term conditions in a community environment- CVAD and drain management- Chemotherapy disconnect- Holistic assessments- Post operative care following discharge- Wound clinic service- Venous blood samples

Education & Qualifications
Essential: - RN1; - Registered with NMC
Skills, Knowledge & Abilities
Essential: - Health and Safety awareness; - Good Computer skills particularly Microsoft packages
Desirable: - Wound Care; - Palliative Care; - Long term conditions
