As a highly skilled NMC Registered nurse you will work in collaboration with the muti-disciplinary team and colleagues to:
Promote independence and support patient centred care planning.
Provide care directly to patients on the caseload with a range of nursing needs (palliative & end of life care, long term condition management, medication administration, complex wound care and bladder & bowel management)
Work autonomously, in line with your scope of practice, competence and policies.
Handover patient information to the wider team, document patient information effectively.
Escalation of patients' needs as appropriate.
Lone working with the support from the wider team.

Qualifications
Essential: - Degree in nursing or equivalent; - Practice Supervisor/ Assessor qualification or willingness to work towards; - Current Active NMC Registration
Experience
Essential
- Ability to manage time & caseload effectively
- Relevant clinical experience to the role e.g. assessment skills & wound care
- Supporting service users, families and carers in dealing with people with nursing needs
- Mutidisciplinary team working
- Record keeping
Desirable: - Understanding of autonomous working and escalation process
Personal Attributes
Essential: - Ability to travel over wide geographical area to meet the requirements of the service
Desirable: - Genuine desire to work in community nursing
