You will be responsible for assessing,
planning, implementing and evaluating individualised care for patients under
the direction of a Clinical Team Lead and District Nurse.
You will work in close collaboration with
health and social care professionals within the Neighbourhood to prevent your
patients being admitted to hospital.
You will provide advice and support to
junior staff and students in the delivery of effective, high quality care.
Due to the
nature of the role it is essential that you are able to drive, have a car
suitable for business purposes and a valid driving license.

This role is to
provide nursing care and treatment to adults and their families in their own
home/residential setting within a neighbourhood, as part of the District
Nursing service.
You will deliver skilled nursing care within the framework of
the NMC Code, promoting a healthy lifestyle and self-care where possible.
