SPA is looking for a Community Specialist Practitioner who shares its vision to progress remote clinical decision making within the community and prov...

  • Provide remote urgent assessment, treatment, and diagnosis of clinically deteriorating service users.
  • Triage and evaluate clinical information from examination and history taking and initiate appropriate treatment and/or referral.
  • Signpost appropriately to other services in the identified healthcare pathway, such as specialist services.
  • Provide a service that focuses on achieving and demonstrating overall improvements in health care for people who are complex and frail, with long term conditions who may present with urgent care needs.
  • Provide robust clinical leadership to ensure high quality harm free care.
  • To promote and champion use of digital technology in remote clinical assessment and decision making.
  • Work collaboratively with the other CSPs
  • Be responsible for the provision of relevant clinical training within the team which may include
  • Supporting team performance in key identified areas including frailty and the assessment and management of long term conditions.
  • Support the development of advanced clinical practice within the team through the provision of education and clinical supervision.
  • Facilitate local and organisational audits, clinical investigations and RCAs relevant to clinical practice.
  • Work as an autonomous practitioner, actively using and promoting shared decision making and empowerment of service users, ensuring they have comprehensive, individualised care plans.

Skills, Knowledge & Experience

Essential

  • Expert knowledge of complex case management
  • Developed skills and experience of managing service users with long term conditions and urgent care needs
  • Experience of using clinical examination and diagnostic reasoning skills
  • Experience in a relevant primary care, community or secondary care setting.
  • Ability to work with a high level of autonomy and shared decision making.
  • Ability to effectively communicate complex clinical information.
  • Experience of working across organisational and professional boundaries to improve care, including challenging service delivery and provision.
  • Experience of managing and leading people.

Desirable

  • Experience of remote assessment
  • Experience of clinical risk assessment

Qualification and Training

Essential

  • Successful completion of a relevant post-graduate L7 (or higher) course or appropriate equivalent modules.
  • Evidence of continuous personal and professional development and the commitment to undertake level 7 masters academic learning that supports the skillset required for this role.
  • Knowledge of local, national, strategic and operational policy developments and an awareness of their implications.
  • Willingness to undertake training required for the role.

Desirable

  • Post registration module at master's level.
  • Coaching / Teaching and Assessing qualification / experience.
  • Understanding of the primary care, secondary care and provider interface.
  • Knowledge of legal issues relating to the care of people such as Data Protection Act, Adult Protection procedures and the Mental Capacity Act
