You will be able to demonstrate real enthusiasm for community nursing, alongside a commitment to ongoing professional development.
The team provides nursing care to housebound patients aged 18 and over within their own homes or chosen place of residence. You will work autonomously, supported by the Community Nursing Sister and Team Lead, delivering a range of nursing interventions and patient-centred care.
The service operates 7 days a week from 8am to 8pm, with flexible working available on request to support work-life balance.
A car owner/driver with business-use insurance cover is essential for this role.

- Experience of providing evidence based interventions with patients
- Experience of working in a multi-disciplinary team and able to demonstrate a positive attitude towards it.
- Post-Preceptorship experience
Skills
Essential
- Evidence of good clinical skills and an ability/willingness to share and teach others
- Demonstrate sound caseload management skills, and an ability to work within a shared caseload model.
- Knowledge of medication prescribing and administration practices
- Skills in prioritising own and team workload
- Able to meet deadlines and manage own time efficiently.
- Be able to input onto a data management system.
- Full UK valid driving license and access to a vehicle
Knowledge
Essential
- KnowledgeoAbility to carry out comprehensive risk assessments and risk management plan.
- Knowledge of Tissue Viability or Continence or be willing to develop
- Knowledge of Palliative Care within a community setting or be willing to develop.
- Knowledge of Long term Conditions or be willing to develop
- Ability to assess a wide variety of clinical health needs.
- Plan, implement and evaluate programmes of care.
