Join our Reablement Team as a Community Reablement Assistant (Level 2) and support adults over 18 in Hampshire needing extra care. Our goal is to maxi...
- Provide Person-Centred Care: Conduct initial home visits and independently perform reablement interventions to restore independence including intimate personal care, continence care, and moving and handling.
- Deliver Outcomes: Perform risk assessments, set goals, update reablement plans for Level 1 Assistants and monitor service user progress.
- Support Operational Delivery:Coach and mentor Community Reablement Assistants and assist Team Leaders, Occupational Therapists and Sensory Officers with activities.
