Community Psychiatric Nurse

As a Band 6 CPN your main duties will be -

To provide a defined level of specialist assessment and access to interventions for a diverse patient group and their families under the care programme approach, who present with significant mental health conditions and people with a dementia diagnosis.

To provide realistic timely or occasionally intensive support to patients, assessing & supporting patients within a care home setting.

To demonstrate active clinical and managerial leadership skills within the team, providing education and supervision to junior staff members and other professionals.

Band 6 - Full time 37.5 hours per week Gedling & Hucknall Dementia Outreach Service.

Office hours 9-5, 5 days a week.

We are seeking an enthusiastic and motivated Band 6 Mental Health Nurse to join our MHSOP Community Mental Health Team within the Dementia Outreach Service (DOS) covering the Gedling & Hucknall locality. We provide assessment, treatment and support for service users of any age with a diagnosis of a dementia or people over the age of 65 with a mental health condition.

The service is for patients with a dementia diagnosis. The team provides specialist assessment, therapy & treatment, working to an individually tailored care plan to adopt a flexible, patient-centred approach enabling the older person with mental health difficulties to engage in services available to them and to lead as independent a life as possible.

DOS - The County Dementia Outreach Service Operates as a virtual team operating within the CMHT , delivering person-centred, dementia-friendly care. The service collaborates with patients, families, and care home staff to reduce distress, manage behaviours of concern, prevent inappropriate hospital admissions, and maintain suitable care placements.

There will be an expectation that the team will work closely with the CMHT and the Intensive Home Treatment Teams (IHTTs) in the locality.

There will be an expectation at times that the Band 6 Clinician will be the duty worker for the team.

You will provide Community Mental Health nursing input into the service. You will also provide specialist assessments and provide support and supervision to junior team members.

You will be an experienced Registered Mental Health Nurse and have current NMC registration. You will have proven post qualification experience and have experience of working with this client group and of multi-disciplinary working. You will have a sound knowledge of risk assessment and be able to demonstrate sound evidence-based practice.

A full UK driving license and vehicle for business use is Essential for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.

You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.

Qualifications

Essential: - RMN; - Specialist knowledge of the nursing needs and issues associated with dementia in a residential setting.; - NMC; - Nursing student mentor.

Desirable: - Non-medical prescribing.

Training

Desirable: - Dementia care/person centred care training.

Experience

Desirable: - Care planning within the MCA and best interest decision making experience.; - Community Experience

Knowledge

Desirable: - National and Local Nursing issues

Desirable: - Experience of delivering Learning and development opportunities for others.
