Job Title: Band 6 Community Psychiatric Nurse (CPN) Trust: Essex Partnership University NHS Foundation Trust Location: Recovery & Wellbeing – Taylor Centre, Warrior House, 42–84 Southchurch Road, Road, Southend-on-Sea, Essex, SS1 2LZ Days: Monday to Friday Hours: 09:00 – 17:00 Duration: 4 Weeks (Possible Extension) Start Date: As soon as possible
Description: We are currently recruiting experienced Band 6 Community Psychiatric Nurses (CPNs) to join the Recovery & Wellbeing Team at the Taylor Centre in Southend. This is an excellent opportunity for skilled mental health professionals who are experienced in community mental health and care coordination.
Manage a caseload of up to 35 patients with severe and enduring mental health conditionsAct as Care Coordinator in accordance with the Care Programme Approach (CPA)Complete comprehensive mental health assessments and risk assessmentsDevelop, implement, and review person-centred care plansWork collaboratively with the multidisciplinary team and external agenciesProvide therapeutic interventions and ongoing support to service usersMaintain accurate clinical records and documentation in line with NHS policiesPromote recovery, independence, and positive mental health outcomes
Valid NMC Registration (Registered Mental Health Nurse)Previous Community Psychiatric Nursing (CPN) experienceExperience managing a community mental health caseloadKnowledge of CPA, risk assessment, and safeguarding proceduresUp-to-date mandatory trainingEnhanced DBSAvailable to start ASAP
