Our team focusses on providing excellent patient-centred care to people within their own homes and in clinic settings. Therefore, it is essential that you have a keen interest and experience in this field.

The successful candidate will also have the ability to work as part of a team as well as on their own and be flexible.

If you can demonstrate good communication skills, have the ability to plan and organise competent, evidence-based, clinical care delivery - we want to hear from you!

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and predominently for this role in people's own homes.

During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.

Education, Qualifications and Training

Essential

  • GCSE English Level C/Level 4 or above (or equivalent)

Desirable

  • NVQ 2 Health/Social Care

Experience and Knowledge Required

Essential

  • Worked in a healthcare setting
  • Willing to undertake the Care Certificate
  • Venepuncture trained

Desirable

  • Experience working in community setting

IT skills

Essential

  • IT experience - examples of systems used
  • Willingness to learn to use System 1 EPR

Desirable

  • previous use of electronic patient records

Skills and Attributes

Essential

  • Able to work on own initiative or as part of a team
  • Ability to organise and prioritise
  • Good verbal and written communication skills
