To be responsible for providing clinical support whilst promoting the effective and efficient delivery of high-quality outcome focused service in a person-centred approach.
This is a generic community care role providing support and cover across Plymouth city localities.
To be responsible for providing clinical support whilst promoting the effective and efficient delivery of high-quality service to the LD population.
There is responsibility for participating in reviewing a persons eligibility for Continuing Healthcare funding and is responsible for making consistent and well-informed professional decisions about the determination of CHC funding, ensuring that the national framework is applied, and they adhere to all local policies and procedures.
This role may not be eligible for sponsorship under the Skilled Worker route; please refer to the Direct Gov website for more information with regards to eligibility.

Clinical Responsibilities
- Participating in the undertaking of health needs reviews and health assessments to identify needs and plan care accordingly.
- To identify, plan and manage risk of an individuals care needs, relating to physical health, behavioural and or mental health.
- To be responsible for assessing, planning and making recommendations for future care in partnership with patient, carer and multi-professional agencies including case conferences for reviewing the packages of care and care co-coordinating as appropriate.
- To liaise with Primary and Secondary care agencies to ensure better health outcomes for individuals.
- To work within the Mental Capacity Act and evidence best interest decision making.
- To maintain effective and appropriate communication networks with members of the multi-disciplinary team, the independent sector homes and all stakeholder agencies and organisations to ensure that the individual care needs can be met.
- To contribute to multi-disciplinary team specialist assessments.
- Ability to work as an autonomous practitioner, planning, prioritising and organising workload according to the needs of the service.
- To provide any relevant information and explanation to clients and carers at a level commensurate with their ability to understand using total communication.
- To provide evidence-based advice and guidance to care staff on the management of health needs to patients and signpost to other services.
- To act as a supervisor and/or assessor for students demonstrating high standards of practice and professional conduct.
- To undertake health promotion/education whenever possible to all adults with LD and also to contribute to planned health promotion activities.
- To work with all other agencies with regards to the safeguarding process.
Managerial Responsibilities
- To be responsible for coordinating reviews as per guidelines
- Prioritise caseload to meet competing demands
- To be aware of and abide by the LWSW policies, standards and Clinical Governance initiatives.
- Present confidentiality and be aware of the Data Protection Act, access to health records and consent for assessment process.
- Involvement in continual audit and review of services to determine quality.
- Maintain safe record-keeping and confidential practice, in line with LWSW and professional guidance and be aware of the Data Protection Act, access to health records and consent for assessment process.
- To ensure electronic record keeping policy is adhered to.
Professional Responsibilities
- To maintain a high professional standard of conduct at all times in accordance with NMC/ professional bodies code of professional conduct. Maintain an active status on NMC/ professional bodies register.
- To maintain own re-validation portfolio.
- Assume responsibility for own professional development and personal knowledge. Undertake and contribute to research where appropriate.
- Monitor own performance against agreed objectives through the process of appraisal and personal development plans and professional body regulations.
- Maintain professional expertise by arranging and attending meetings, study days and mandatory training and service training.
- Plan and participate in the supervision and provision of experience for all nurse learners and other personnel as appropriate.
- Participate in regular line management, caseload management and clinical supervision.
- To deliver appropriate training and subsequent support to other staff, students and outside agencies when required.
COMMUNICATIONS AND RELATIONSHIPS
- The post holder will be required to work within the MDT function of the CLDT and may need to be part of wider MDT groups relating to client need.
- The post holder will liaise between other health, social and educational services in assessing, planning and reviewing client care needs/service provision.
- The post holder will also work with other voluntary and secondary care providers.
PHYSICAL DEMANDS OF THE JOB
- Ability to frequently travel independently and in a timely manner between Livewell Southwest sites and community locations including areas not served by public transport.
- Use of keyboard and computer screen for assessments and record keeping daily.

Experience
Essential
- Working in multidisciplinary teams and across health and social care.
- Managing a complex caseload of individuals who pose a risk to themselves and society.
- Experience of working across organisational and inter-agency boundaries.
- Experience of working with people with learning disabilities / mental health issues whose behaviour poses a risk.
Desirable
- Experience in mentoring students.
- Working in close contact with ICB and local commissioners.

- Able to demonstrate understanding and competency in assessment, care planning and review.
- Good knowledge and understanding of the, Mental Capacity Act, Deprivation of Liberty and reasonable adjustments.
