As a Band 6 Community Nurse / District Nurse, you will play a key role in delivering high-quality, person-centred care within the community setting. Working as part of a multi-professional team, you will be responsible for the assessment, planning, coordination, and delivery of clinical care that meets the needs of patients and the service. You will work autonomously within your scope of practice, using your clinical expertise, professional judgement, and evidence-based knowledge to make informed decisions and deliver safe, effective care. As a registered practitioner, you will be accountable for your actions, recognising the limits of your competence and seeking support where appropriate. The role also includes supervising and supporting junior staff, support workers, and students, helping to develop future healthcare professionals. You will actively contribute to quality improvement initiatives, service development, and the ongoing enhancement of patient outcomes, ensuring care is delivered in line with professional standards, organisational values, and best practice. Deliver compassionate, person-centred nursing care by assessing, planning, implementing, and evaluating care interventions that meet the individual needs of patients and their carers.Manage a diverse caseload, including patients with complex health needs, ensuring care is coordinated, responsive, and focused on achieving positive outcomes and promoting independence.Conduct holistic assessments using appropriate clinical tools, identifying risks, safeguarding concerns, and changes in patient condition, taking timely action and escalating where required.Undertake a range of clinical procedures within scope of practice, including observations, venepuncture, catheter care, ECGs, and other competency-based interventions relevant to the service.Work autonomously and as part of a multi-disciplinary team, effectively prioritising workload, coordinating care, and delegating tasks appropriately to ensure safe and effective service delivery.Provide health promotion, education, and advice to patients and carers, empowering them to make informed decisions and actively participate in the management of their health and wellbeing.Build effective relationships with patients, families, colleagues, and partner organisations, communicating complex or sensitive information clearly and professionally.Maintain high standards of clinical practice by adhering to NMC standards, accurately documenting care, supporting quality improvement initiatives, and raising concerns when care quality, safety, or patient outcomes may be compromised. Person SpecificationEssential Criteria

Registered Nurse (Level 1) with current NMC registration and a commitment to practising in line with the NMC Code of Conduct.Significant post-registration nursing experience, ideally within community, district nursing, or related healthcare settings.Proven ability to lead, support, and motivate colleagues while contributing positively to team performance.Experience managing and coordinating complex caseloads, including patients with long-term conditions, palliative care needs, and multiple co-morbidities.Strong clinical assessment and decision-making skills, with the ability to work autonomously and prioritise effectively in a fast-paced environment.Demonstrable expertise in a broad range of clinical skills, including:

End-of-life care, symptom management, and syringe driver administrationWound assessment and managementDoppler assessments and compression bandagingVenepunctureMale, female, and suprapubic catheterisationMedication administration, including intravenous therapiesPressure ulcer prevention and management

Excellent verbal and written communication skills, with the ability to build effective relationships with patients, carers, colleagues, and partner organisations.Ability to remain calm, professional, and resilient when managing challenging or complex situations.Strong organisational skills, with the ability to effectively manage time, workload, and resources.Commitment to supporting learning and development, with mentoring, teaching experience, or a willingness to work towards a recognised qualification.Full UK driving licence and access to a vehicle to meet the travel requirements of the role.Ability to work both independently and collaboratively, adhering to local lone working policies and procedures.
