Community Neuro Occupational Therapist

• Work as an active member of a multidisciplinary team specialising in stroke and neurological rehabilitation.
• Deliver specialist Occupational Therapy assessment, treatment and rehabilitation for adults with complex neurological conditions.
• Manage a caseload of patients with complex physical, cognitive and emotional needs.
• Develop and implement person-centred rehabilitation plans that support independence and maximise recovery.
• Contribute to timely, coordinated and safe discharge planning in partnership with patients, carers and relevant professionals.
• Support the development of the Occupational Therapy service and wider team to promote integrated, evidence-based practice.
• Provide specialist advice, guidance and education to health and social care professionals, patients, carers and families.
• Work collaboratively with multidisciplinary colleagues to deliver high-quality community rehabilitation services.
• Support and supervise junior staff, support workers and students, contributing to their learning and development.
• Maintain professional standards through reflective practice, continuous professional development and participation in service improvement activities.

Essential

• Diploma/Degree in Occupational Therapy
• Current AHP with UK HCPC registration
• Post-graduate training in Approaches to Rehabilitation / Cognitive rehabilitation / Neurological rehabilitation or equivalent.
• Attendance at specialist short courses or equivalent CPD activities within the specialist areas of Neurology

• Masters level study

Essential

• A minimum of 4-5 years post-graduate experience with a minimum of 2 years in older peoples / neurological rehabilitation services
• Experience post registration as a Band 6 occupational therapist in neurology (possibly as part of a rotation)
• Experience in working with people with complex health and social issues.
• Experience in working collaboratively across agencies in planning care for people
• Experience in planning complex discharges, co-ordinating services and resources.
• Experience in working with adults in their own home and working in community services

Desirable

• Rotational experience at Band 5 or Band 6 level in acute trusts and stroke services.

Essential

• Knowledge of role(s) of Allied Health Professionals and interdisciplinary team working
• Knowledge and understanding of complex medical conditions including, neurology, multi-pathologies and falls
• Ability to apply core occupational therapy skills across a range of clinical conditions within a client-centred, goal setting
• Knowledge of moving and handling legislation and procedures
• Excellent written and verbal communication skills
• Ability to problem solve in a creative manner and to make clear and realistic decisions with clients and their carers.

Desirable

• Knowledge and experience in research and audit
