Community IV Nurse - Hampstead

As a Community IV Nurse, you will provide specialist

nursing care to patients receiving IV therapies and Parenteral Nutrition in

their own homes and community settings. You will support patients throughout

their treatment journey, ensuring safe and effective care while promoting

independence and confidence in managing their treatment.

This role offers the opportunity to work autonomously

while developing strong relationships with patients, carers and healthcare

professionals. You will be part of a collaborative multidisciplinary team

dedicated to achieving the best possible outcomes for patients.

Deliver safe, high-quality,

patient-centred care in patients' homes and community settings.

(PN), IV therapies and clinical interventions in line with SOPs and

professional standards.

Assess patients, identify changes in

condition and escalate clinical concerns appropriately.

Support treatment initiation, ongoing care

and transitions from hospital to home.

Train, educate and empower patients,

families and carers to safely manage treatment and build independence.

Provide clinical advice and support while

building strong relationships with patients and healthcare professionals.

Work collaboratively with NHS colleagues,

Senior Homecare Nurses, Clinical Nurse Managers and wider teams to ensure

continuity of care.

Maintain accurate clinical records and

work in line with Clinical Governance, NMC standards and Inizio policies.

Support patient safety, quality

improvement, service development and achievement of programme KPIs.

Maintain clinical competence through

training, CPD, reflective practice and evidence-based care.Maintain accurate
