An exciting opportunity has arisen for a Band 6 Heart Failure Clinical Nurse Specialist to join the community Heart Failure team in Pembrokeshire. We ...
- Manage a defined caseload of patients with complex heart failure, using evidence-based and person-centred approaches
- Undertake comprehensive, holistic assessments and develop, implement and evaluate care plans across the primary and secondary care interface
- Work autonomously across a range of settings including patients' homes, clinics and hospital environments
- Provide specialist advice, education and support to patients, families and carers to promote self-management and improve outcomes
- Deliver education and support to colleagues to enhance knowledge and standards of heart failure care
- Work collaboratively with multidisciplinary teams including cardiology, general practice and community services
- Maintain accurate and timely clinical documentation in line with professional and organisational standards
- Contribute to reducing avoidable hospital admissions through proactive management and early intervention
- Actively engage in ongoing professional development, including undertaking relevant post-registration education
- Demonstrate and actively promote the Health Board's values in all aspects of practice

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.
We have over 13,000 staff and together we provide primary, community, in-hospital, mental health and learning disabilities services.
We work in partnership with the three local authorities, as well as public, private and third sector colleagues, including our valued team of volunteers.
Our services are provided in
- Four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest
- Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire
- Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings
- 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 99 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers
- Numerous mental health and learning disabilities services
Qualifications and Knowledge
Essential
- Registered Nurse (Part 1)
- Current NMC Registration
- Bachelors Degree
- Specialist post graduate qualification or equivalent experience
- Proficient level of knowledge in clinical speciality
- Knowledge of service improvement
- Able to undertake a literature search and critically appraise research papers
Desirable
- Advanced clinical skills
- Teaching qualification (or equivalent experience)
- Non-medical Prescribing (v300)
Experience
Essential
- Relevant experience at Band 5
- Experience in the relevant specialty
- Experience of working in a multi-professional environment
- Experience of working with preregistration nursing/midwifery students in a mentorship role
- Experience of undertaking audit
Desirable
- Band 5 experience
- Experience of participating in research
- Experience of participating in practice change
Other
Essential
- Flexibility in working pattern to ensure maximum visibility of CNS team.
- Ability to travel to all areas of the UHB in a timely manner
