Community Dietitian

As a Community Dietitian for HCRG Care Group, your role is crucial in facilitating nutritional health in the community. You'll be key in offering expert dietetic assessment, treatment, and management to diverse patients in various community settings across Wiltshire and Swindon. Working collaboratively with healthcare teams, you'll ensure integrated and patient-centred care. Your responsibilities will include managing varied clinical caseloads, designing evidence-based nutritional care plans, and providing specialist nutrition-related advice. Additionally, you will contribute to ongoing service development, training initiatives, and foster a culture of continuous improvement.
