As a Community Dietitian for HCRG Care Group, your role is crucial in facilitating nutritional health in the community. You'll be key in offering expert dietetic assessment, treatment, and management to diverse patients in various community settings across Wiltshire and Swindon. Working collaboratively with healthcare teams, you'll ensure integrated and patient-centred care. Your responsibilities will include managing varied clinical caseloads, designing evidence-based nutritional care plans, and providing specialist nutrition-related advice. Additionally, you will contribute to ongoing service development, training initiatives, and foster a culture of continuous improvement. This permanent role based at Chippenham Community Hospital offers a Band 5 AfC Salary ranging from £32,073 to £39,043 FTE, includes NHS Pension, and provides access to a suite of rewards such as discounts, cashback offers, and comprehensive well-being support.

Qualifications

Essential

  • Degree in Nutrition and Dietetics (or equivalent qualification acceptable for HCPC registration). Evidence of ongoing Continuing Professional Development (CPD). Experience working within inpatient and outpatient dietetic services in an NHS setting. Experience using MUST or a similar nutrition screening tool. Experience working effectively within a multidisciplinary team. Excellent verbal and written communication skills. Strong presentation and education skills. Effective problem-solving, organisation, and time management skills. Ability to work independently using initiative, as well as collaboratively as part of a team. Good IT skills and confidence using digital systems.
