Your responsibilities will include

  • Delivering seamless nursing care to housebound patients.
  • Coordinating care to prevent unnecessary hospital admissions.
  • Promoting self-care for patients with long-term conditions.
  • Contributing to health promotion and innovative projects that improve patient outcomes.

Our service operates between 8am and 9pm, with core hours of 8am-5pm. The post holder will be expected to be able to travel independently across the borough.

This role is not applicable for sponsorship and candidates must be able to demonstrate their right to work in the UK.
