Band 6 Clinical Triage Practitioner
We are seeking three experienced Band 6 Clinical Triage Practitioners to join a dynamic multidisciplinary team supporting services across the Bath and North East Somerset, Swindon and Wiltshire (BSW) area. The Role As a Clinical Triage Practitioner, you will be responsible for managing referrals and providing clinical triage for healthcare professionals across BSW. Working within a fast-paced environment, you will assess referrals and ensure patients are directed into the most appropriate care pathway, including:
Hospital at HomeIntegrated Neighbourhood TeamsUrgent Care ResponseOther community-based services You will also work through referrals on the waiting list, helping to ensure patients receive timely and appropriate care. Requirements To be successful in this role, you will have:
Registered Nurse status with relevant Band 6 experience.Previous clinical triage experience within a multidisciplinary setting.Excellent communication and clinical decision-making skills.Strong IT skills with experience using SystmOne.A valid NHS Smartcard.The ability to work effectively as part of a multidisciplinary team and make sound clinical judgements. Location There are three available base locations:
SwindonPeasedownGreenway Hybrid working may be available, subject to service requirements. Working Hours This role operates on a shift basis. Available shift patterns include:
