Job description

Take your clinical career in a new, flexible direction Are you an experienced clinically-registered professional looking for a flexible role where you...

As a Bank Clinical Practitioner within our All-Age Complex and Continuing Care (AACCC) service, you'll use your clinical knowledge and experience flexibly to make a real difference to people living with complex health and care needs. This is an opportunity to take your career in a new direction, combining clinical assessment, professional judgement and collaborative working to ensure people receive the right care and support through flexible bank assignments. At the heart of your bank role will be undertaking NHS Continuing Healthcare (CHC) assessments and reviews, working closely with individuals, their families and multidisciplinary teams to understand their needs and make evidence-based recommendations in line with the National Framework. You will have opportunities to work across the wider CHC pathway, including Fast Track, care planning, clinical commissioning, case management and personal health budgets. No two days will be quite the same. You'll work with people in a variety of settings on a flexible basis, navigating complex clinical situations and bringing information to influence different professionals to support person-centred decision making. You'll advocate for individuals, work collaboratively with health and social care partners and use your clinical expertise to help ensure care is safe, appropriate and responsive to changing needs.

Holistic CHC Assessment: Autonomously coordinate and complete Checklists, Decision Support Tools (DSTs), and reviews for adults and children.

Flexible Pathway Delivery: Adapt quickly to support Fast Track cases, care planning, clinical commissioning, case management, and Personal Health Budgets (PHBs).

MDT Collaboration & Advocacy: Chair and facilitate complex multidisciplinary meetings, advocating for individuals while collaborating with health and social care partners.

Evidence-Based Recommendations: Synthesize complex clinical information to present robust, defensible eligibility recommendations.

Quality & Safety Assurance: Navigate fluid clinical environments to ensure commissioned care is safe, appropriate, and responsive to changing patient needs.
