The successful candidate will work closely with our existing pharmacy
team, GPs and wider multidisciplinary team to support safe, effective and
evidence-based prescribing and medicines optimisation.
The successful candidate will work closely with our existing pharmacy team, GPs and wider multidisciplinary team to support safe, effective and evidence-based prescribing and medicines optimisation.
Depending on experience and prescribing qualifications, the role may include:
- Undertaking structured and clinical medication reviews
- Supporting patients with polypharmacy and complex medication needs
- Managing medicines following discharge from hospital and other care settings
- Medicines reconciliation and resolving medication discrepancies
- Supporting repeat prescribing and medication reauthorisation
- Reviewing appropriate monitoring requirements and arranging investigations
- Providing medicines advice to patients and members of the clinical team
- Supporting the management of long-term conditions
- Reviewing patients at increased risk of medicines-related harm
- Supporting safe prescribing in care homes and for vulnerable patients
- Responding to local and national medicines safety guidance
- Contributing to prescribing audits, quality improvement and service development
- Supporting the implementation of NICE and local prescribing guidance
- Working collaboratively with community pharmacy, secondary care and other healthcare professionals
For an appropriately qualified Independent Prescriber, the role may also include making prescribing changes, managing appropriate clinical caseloads and running medication or long-term condition clinics within agreed scope of practice. These responsibilities reflect the existing advanced pharmacist role within the practice.
Heart Failure
We are particularly keen to hear from applicants with an interest or experience in Heart Failure.
For the right candidate, there may be an opportunity to develop this interest further and contribute to the medication optimisation and ongoing management of patients with Heart Failure, working closely with GP and wider clinical colleagues.
Previous Heart Failure experience is desirable but not essential.
Jorvik Gillygate Practice is a large, well-established GMS training practice serving a diverse patient population across central York.
Our multidisciplinary team includes GP Partners, Salaried GPs, nurses, advanced practitioners, pharmacists, First Contact Physiotherapists, mental health practitioners, care coordinators, social prescribers and an experienced administrative team.

- Relevant post-registration pharmacy experience
- Good knowledge of medicines optimisation and applied therapeutics
- Excellent communication and interpersonal skills
- Ability to work effectively both independently and as part of a multidisciplinary team
- Commitment to safe, evidence-based prescribing and patient-centred care
- Ability to manage and prioritise own workload
