Provide clinical support to the surgeries within the Bridgwater Bay Primary Care Network as an Independent Pharmacist Prescriber, consulting with patients through face-to-face clinics, telephone appointments and video consultations.,
Provide clinical support to
patients in care and nursing homes as well as the wider community.
Carry out structured medication
reviews for all patient cohorts.
Support the PCN practices to
improve medicines reconciliation on discharge, formulary compliance, repeat
prescribing management and medication reviews.
Support Bridgwater Bay PCN and
Somerset ICB plans to reduce medicines waste in primary care.
Provide clinical support to the

medicines information to patients, carers and healthcare professionals.
To provide information, advice
and guidance on the safe, rational, effective and economical use of medicines.
Undertake clinical audit and
other outcome measures to support patient care and safety.
Create and implement medicine
related guidelines, policies and SOPs as appropriate to the different service
areas.
Provide and receive complex,
sensitive information and communication relating to pharmaceutical care of patients.
Provide prescribing service
within sphere of competency to patients in primary care, as a pharmacist
independent prescriber.
To develop, advise, agree and
implement action plans at a practice level, monitor the effect of changes made
and ensure that any benefits are maintained in the longer term. This will
Reviewing prescribing data for
the practices to highlight areas where there is scope for improving the
clinical effectiveness of prescribing.
Obtaining agreement with the
practices to implement medicines optimisation projects with the overall aim of
following national prescribing guidance and cost effective prescribing.
Advising GPs and other health
professionals on cost effective prescribing and aiding rationalisation of drug
choice via formulary/guideline/protocol development.
Record clinical activity,
interventions, KPIs and to monitor the outcome of interventions.
Provide training on medicines on
a regular basis to GP surgery staff, patients and carers in all relevant
aspects of medicine use.
Support the pharmacy technicians
working within the PCN.
To maintain knowledge of
developments in pharmacy practice on a regular basis in line with General
To fulfil the professional
requirements of the Code of Ethics and Standards of Good professional Practice
of the GPhC
The post holder will be
responsible to the SHS Lead Pharmacist and Primary Care Network Clinical Director
but will be expected to use their own initiative to make complex decisions
within defined parameters with little supervision.
Requirement to participate in
the national training scheme for pharmacists in primary care.
(PCN) is the largest PCN in Somerset with 9 GP practices and a diverse
population spread across town and rural locations.
As a PCN we are forward
thinking, innovate and driven to deliver the best patient care for our
population. This includes health population management, and this role ties in
with supporting that and tracking the improvements we can make to patients
lives.
In partnership with Somerset NHS Foundation Trust, we have developed a joint venture to open a Health and Wellbeing Hub at the former Victoria Park Medical Centre. This flagship hub, the first of its kind, brings together Primary and Secondary Care under one roof to provide joined-up services and support for the Bridgwater Bay community.
Qualifications
Essential
- Professional registration with GPhC
- Certificate or MSc/Diploma Pharmacy Practice (or equivalent experience)
- Evidence of continued professional development (CPD)
Experience
Essential
- Experience of working as a qualified, registered pharmacist in primary care, community or hospital pharmacy
- At least 2 year post-registration pharmacy experience
- Clinical pharmacy experience
- Clinical audit experience
- Knowledge and understanding of Primary Care
Knowledge

- Knowledge of Teamnet
- Understanding of, and commitment to, equality, diversity and inclusion
- Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities, individuals, their families and carers

- Knowledge of how the NHS works, including primary care and PCNs
- Ability to recognise and work within limits of competence and seek advice when needed
- Understanding of the needs of children with mental and emotional needs/ children with disabilities long term conditions particularly in relation to promoting their independence
- Knowledge of long term conditions and the complexities involved: medical, physical, emotional and social
- Knowledge of, and ability to work to policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety
