Homecare Clinical Nurse Advisor - TPN (Outpatient Services)Location: Thames Valley Contract: Permanent, full timeHours: 40 hours per week split shifts 7am-12 and 5pm-10pm We are seeking an experienced and compassionate Clinical Nurse Advisor to support patients receiving Total Parenteral Nutrition (TPN) through a specialist outpatient homecare service. This role focuses on delivering high-quality clinical care, education, and ongoing support to patients living with complex nutritional needs, enabling them to safely and confidently manage TPN therapy in their own homes and community settings. The role Working across a specialist outpatient programme, you will support patients from initiation through to long-term management of TPN therapy. You will provide hands-on clinical care, structured education, and ongoing follow-up, while working closely with multidisciplinary teams to ensure safe, effective, and patient-centred delivery. You will be trusted to manage your own caseload and contribute to service quality through audit activity, service review, and continuous improvement.
Clinical care & patient educationDeliver high-quality clinical care in line with agreed Service Operating ProceduresProvide structured education and ongoing support to patients, families, and carers receiving TPN therapySupport patients within their own homes and approved community settingsAssist with therapy initiation, follow-up visits, and ongoing assessmentPromote patient confidence, independence, and adherence to therapyMaintain and manage all clinical equipment and materialsCollaboration & service deliveryBuild strong working relationships with dietitians, nutrition nurses, pharmacists, and wider healthcare teamsSupport outpatient pathways and continuity of care between hospital and homeDeliver services in line with agreed KPIs and service expectationsIdentify, manage, and escalate adverse events in line with policyComplete all clinical documentation, activity reporting, and administration accurately and on timeQuality, safety & governanceMaintain high standards of documentation and clinical governanceComplete all mandatory and programme-specific trainingAdhere to safeguarding, pharmacovigilance, health & safety, and regulatory requirementsMaintain professional registration and ongoing CPD (including NMC/NMBI revalidation)Work in line with professional codes of practice
What we're looking for Essential
Registered Nurse with current NMC or NMBI registrationMinimum 1 year post-registration nursing experienceFull UK or Irish driving licenceExcellent communication skills and the ability to build trust with patients and familiesStrong organisational skills with the ability to manage a caseload independentlyA patient-centred approach and interest in long-term condition managementConfident using digital systems and Microsoft Office applicationsSatisfactory enhanced DBS (or equivalent) and pre-employment medical checks
Desirable
