Clinical Lead for Bed Based Care

As a valued member of the leadership team, you will have the opportunity to influence strategic and operational direction, support workforce development, and contribute to the continued success and evolution of the Intermediate Bed-Based Care Service, ensuring it remains responsive to the needs of patients, carers, and the wider healthcare system.

They will support, manage and develop

staff, ensuring the workforce has the skills and capability required to meet

organisational objectives and deliver excellent patient outcomes.

A key

responsibility is leading the development and implementation of an Integrated

Multidisciplinary Team (MDT) Model of Care, working closely with system

partners to improve integration, patient flow and service effectiveness.

role involves maintaining oversight of hospital discharge policies and transfer

of care processes, ensuring compliance with national and local requirements and

embedding these within operational practice.

This is an

excellent opportunity to provide professional leadership, service development

and support the introduction of new initiatives within a 28 bedded Intermediate

Care ward.

The role is ideal

for someone who is a strong team player who is able to foster strong inclusive

leadership. We are looking for a practitioner who is highly motivated to lead

on our continued commitment to providing innovative quality improvement

programmes to ensure high standards of patient care.

This is a part time role working 30 hours per week.

Qualifications
