Job summary To provide high-quality, proactive and person-centered clinical assessment, intervention and care coordination for care home residents with moderate and severe frailty and/or complex health needs. The post holder will act as the clinical delivery arm of the multidisciplinary Care Home Support Team, translating MDT decisions and anticipatory care plans into timely hands-on care within the care home.
