The successful applicant will receive direct line management and will be delegated team work priorities from the Home First Shift Leads. The post holder will be enthusiastic, passionate and a dynamic individual. You will need to be motivated, flexible and an effective team player, in order to prioritise and implement effective negotiation and communication skills.

Review all daily Home First referrals with the Locality Teams and ensure that all patients are allocated the appropriate care calls within the discharge to assess appropriate referral timescale.

Ensure that all Home First referrals from the acute portals are immediately sourced and prioritised throughout the working day.

Attend all CDT training and briefing sessions as the system evolves.

  • Ensure that any and all updates recorded in CDT are accurate before entering them. Ensure that any identified mistakes are corrected immediately.

  • Midlands Partnership University NHS Foundation Trust.
  • North Staffordshire Combined Healthcare NHS Trust.

  • Staffordshire and Stoke-on-Trent Integrated Care Board.

Qualifications

Essential

  • GCSEs or equivalent Certificates/Qualifications in Administration
  • Ability to effectively maintain, update and sustain systems
  • Excellent communication skills
  • Demonstrate a commitment to continuous professional development

Desirable

  • NVQ Level 3 in Care or equivalent qualification or experience.

Experience

Essential

  • Significant experience of working in health / social care environment
  • Experience of working within the constraints of legislation, regulation, policy and procedures
  • Experience of delivering improvement to consistently achieve and exceed personal and team objectives
  • Experience and ability to work collaboratively
  • Experience of work planning & allocation, effective coordinator of allocated care calls to meet the service delivery
  • Significant experience of supporting and enabling others to undertake day to day scheduled activities

Desirable

  • Ability to liaise with multi-disciplinary professionals
  • Experience of Health and Social Care working
  • Experience of working alongside primary care teams including the voluntary and third sector.

Knowledge, Skills and Attributes

Essential

  • Experience of working alongside primary care teams including the voluntary and third sector.
  • Knowledge of the Care Act
  • CQC fundamental standards and registered service requirements
  • Effective problem solving skills (in order to be able to respond independently to unexpected situations)
  • The ability to work as part of a multi-disciplinary team
  • Effective interpersonal skills; listening, verbal and written communication skills with colleagues and across a diverse service user base
  • Effective rostering skills in a fast paced, changing working environment

Desirable

  • Understand relevant policies, processes and procedures relevant to the role
  • Have a good understanding of common Health Conditions and how they affect the delivery of care

Personal Attributes

Essential

  • Working across various locations and community settings and have the ability to travel within a geographical area to meet the requirements of the post.
  • Flexibility with working hours to enable 7 day services
